VeriSim: A Configurable Framework for Stress-Testing Medical AI Under Patient Communication Noise
A patient-simulation framework that injects controllable noise along six clinically grounded communication dimensions while preserving the underlying medical record, with a verifier that extracts atomic claims from each simulated utterance and checks them against a UMLS-grounded index. Across seven open-weight LLMs, realistic communication noise reduces diagnostic accuracy by 15 to 25 percentage points and lengthens conversations by 34 to 55%, with 7-to-8B models degrading 1.4 times more than 70B-plus models; a physician and a nurse rated the simulated conversations highly on truth, realism, clinical utility and noise fidelity.
